Columbus East Chiropractic is happy to provide online access to our forms so that they may be completed in the comfort of your home or office prior to your visit. Since the New Patient Information and Questionnaire forms cover detailed health history, please plan on spending at least 10 – 20 minutes filling them out.
You have the option of either printing the forms and completing them by hand, or filling them out within your web browser. Whichever method you choose, please be sure to bring the completed forms with you to your appointment, or email them to us at columbuseast480@gmail.com.
